Medical Care

Four health plans are available for you to choose from. Coverage is provided through either the First Choice Health Network or Premera’s Foundation Dimensions. All plans include prescription coverage with a mail order option. 

Prescription Drugs

Prescription drug coverage is included in the medical plans. The copays vary from $5 for generic (under the PPO plans) to $50 for non-preferred brands. Mail order is available in all plans.

Vision

In the PPO plans, the core vision plan includes coverage for hardware and annual visits. A buy-up option is available for an additional cost. Evergreen pays 100% of the employee only coverage in the core vision plan and subsidizes employee only coverage in the buy-up plan. Employees pay for dependent coverage. The Premera Foundation plan provides for annual examinations.

Dental

Evergreen provides the DeltaPreferred Option plan from Washington Dental Service. Evergreen pays 100% of the employee only coverage and subsidizes a portion of the cost for dependent coverage. A buy-up option is available for an additional cost.

Lab Work/Preventive Medicine

All employees may receive annually at no charge:  CBC, urinalysis, pap test and Hepatitis B vaccine.
