Self pay accounts are payable upon receipt of the statement/bill, unless other arrangements have been made. I
f you are unable to pay the amount in full, please call Customer Service at 425.899.3292 immediately. A representative will gladly work with you to help you meet your responsibility.
Financial assistance is available for qualifying patients.
