Seventy-one-year-old Gordy Cheeley was struggling in spring 2000. He'd already undergone two invasive heart procedures - including a triple bypass - and was working hard in cardiac rehab. But he still wasn't feeling well.

The goal is to help patients feel better and stay out of the hospital. This usually requires a team approach, because chronic heart failure is a life long disease state, and day to day management is essential.
We recognize the effort this takes, and we strive to partner with patients and their families to help along the way.
The program was recognized with a 2005 Award of Excellence in Healthcare Quality from Qualis Health, which recognizes innovations that make measurable improvements in outcomes for patients.
Evergreen is also one of the leaders in a local task force of healthcare professional leaders from multiple hospitals who are striving to improve patient safety throughout the continuum of care for heart failure.
