Individual Consultations

We offer individual consultations with our certified diabetes educators to anyone unable to attend classes, as well as for those requiring specific education not provided in the classroom setting. Reasons you might want an individual consultation include, but are not limited to:

• learning to give an injection
• pregnancy with high blood sugar (gestational diabetes)
• specific dietary requirements
• matching carbohydrate intake with insulin dosing

Insurance and Payment Information

Our individual consultations are covered by most insurance providers, including Medicare and Medicaid. Please contact your insurance provider to discuss the coverage available to you.

Evergreen offers resources for those without insurance coverage. Please call 425.899.3008 for information about the resources you may be eligible for.

Pricing Information

To meet with a Registered Nurse:
$280 per hour; additional time is available in 30 minute increments
NOTE: New price of $240 per hour effective in January 2011

To meet with a Registered Dietitian:
$224 per hour; additional time is available in 15 minute increments
NOTE: New price of $246 per hour effective in January 2011; followup consults available for $232 per hour, with time available in 15 minute increments.

• Check with your insurance provider to learn how much diabetes education coverage you may have available.
• Get a referral from your provider prior to your appointment; we are happy to help with this when you call.
