Nursing is able to assess effectiveness of medications and treatment for specific illnesses. Some specialties include pain control, wound treatment, diabetic management and catheter care. Patient/family education is provided to maximize wellness and self-help.

Physical and Occupational Therapy can maximize independence through safety assessments, home modifications and strengthening and home exercise programs to assist in your recovery from physical limitations due to surgery or illness.

Speech Therapy and language rehabilitation assess and treat swallowing and speech difficulties and problems with memory and judgment.

Social Work offers assessment and counseling regarding financial and support needs along with referrals to community resources and planning for long-term care.

Home Health Aides provide assistance with bathing and personal hygiene.

Mental Health services provide counseling and assistance with anxiety, depression and other psychological problems.

How Does Home Health Work with My Doctor?

We are an extension of the medical care directed by your doctor.

Who Pays for Home Health Care?

Medicare, Medicaid and most insurance plans cover home health services. Evergreen will bill your insurance company directly for services. We will help answer any questions you may have about your coverage.
