Dr. Griffith is a Movement Disorders neurologist, fellowship-trained at Harvard-affiliated Beth Israel Deaconess Medical Center in Boston and a diplomate of the American Board of Psychiatry and Neurology. Her expertise includes Parkinson's Disease and other movement disorders, patient selection for deep brain stimulation surgery and post-surgical management of deep brain stimulators, as well as the administration of Botox injections for dystonia, blepharospasm and hemifacial spasm. Dr. Griffith is a principal investigator for clinical research trials at the Center.

Dr. Agarwal is a board certified neurologist, fellowship-trained in Movement Disorders at The Neurological Institute, Columbia-Presbyterian Medical Center. Her interests include Parkinson's disease, dystonia, essential tremor, restless legs syndrome, deep brain stimulation for dystonia, tremor and Parkinson's disease, cerebellar ataxia, Huntington's disease, Botox injections for spasticity and dystonia, tic/Tourette's syndrome, and myoclonus. Dr. Agarwal has been the principal investigator on numerous clinical trials and was previously Director of Clinical Research at The Colorado Neurological Institute. She is also a clinical assistant professor at the University of Washington.

Dr. Burdick is a fellowship-trained Movement Disorders specialist, having completed neurology residency at the Harvard-affiliated Beth Israel Deaconess Medical Center in Boston and then fellowship at the University of Washington. His interests include Parkinson's disease; counseling for and management of deep brain stimulation (DBS) for Parkinson’s disease, dystonia, or tremor; Botox injections for dystonia, hemifacial spasm, blepharospasm, and spasticity; essential tremor; ataxia; Huntington's disease and other forms of chorea; tic disorders, including Tourette’s syndrome; and myoclonus. He has also been an investigator in several research studies.

Amy Cole is a registered nurse focusing on geriatrics, psychiatric nursing and oncology, in both inpatient and home health environments. She is 2006 graduate of Carroll College, with with a Bachelor of Arts degree in nursing. She was awarded membership in Sigma Theta Tau for her academic achievements.
Marjie Kay Thomas graduated from Lake Washington Technical College in June 1995 as a Certified Medial Assistant. Marjie Kay has a twelve year history as a medical assistant specializing in movement disorders, geriatrics and primary care.
Penny Bridges has 15 years of medical assistant experience specializing in neurology (movement disorders), urology and cardiology. She graduated from Eton Technical Institute in June 1995 as a medical assistant, and joined the Parkinson's Center in 2008.
Jeffrey Shaw has been a staff Neuropsychologist with Evergreen Hospital since 2001. He works extensively with individuals diagnosed with Parkinson's disease, Multiple Sclerosis, strokes, and varied neurological difficulties. Although Dr. Shaw specialized into the field of Neuropsychology (thorough evaluation of thinking skills), he also provides counseling and psychotherapy services. He teaches in the graduate Psychology program at Seattle Pacific University.

Ann Zylstra is the Supervisor of Evergreen's out patient rehabilitation services and the Center's lead physical therapist, with more than 21 years experience developing rehabilitation programs for patients with Parkinson's and other neurological problems that affect balance and gait. Ann is the co-president of the Washington Chapter of the APDA. Ann is certified in vestibular rehabilitation.

Evelyn Donascimento is a physical therapy assistant and earned her Associate of Science in Physical Therapy at Miami Dade Community College. Evelyn has twelve years experience working with persons diagnosed with Parkinson's disease, multiple sclerosis, cerebrovascular accidents, traumatic brain injury, dementia and other neurological disorders.
Kay Ballen is the lead Occupational Therapist at the Booth Gardner Parkinson's Care Center. Kay has over 20 years experience working with neurologic diseases that affect the use of upper extremities, vision, cognition which include everyday activities such as eating, dressing, home chores, working, and driving.
Rae Gamble is a licensed occupational therapist who obtained her degree in South Africa in 1992. She has worked in South Africa and England. Rae has an interest in working with patients with all types of neurological disorders as well as hand injuries.
Beth Edwards is a licensed physical therapist and earned her Doctorate of Physical Therapy degree from the University of Washington in 2006. She trained in neurological and vestibular rehabilitation and treats patients with Parkinson's, MS, stroke, brain injury and other neurological impairments as well as vestibular conditions.
Lisa Ireland is an outpatient speech therapist serving adults with neurological disorders. She holds certification from LSVT Loud (Lee Silverman Voice Treatment) and Voicewave Technology, manufacturer of the Speech Enhancer SGD. Lisa also performs modified barium swallow studies for patients from the region.
