Online Preregistration

If you are a first time patient at Evergreen and already have made your appointment, you can use our online preregistration form.

Maternity patients may preregister using your expected due date.

Our secure preregistration form offers you the convenience of providing your information at your leisure and in the comfort of your own home or office. Additionally, it saves you time the day of your visit because Evergreen registration staff will not need to obtain as much information from you upon your admission.

Please note:

  • The online pre-admission form must be completed at least 72 hours prior to your scheduled appointment.
  • First Time Patients Only
  • If you are within 72 hours, please contact our staff at 425.899.2722 to complete your pre-registration.

The Evergreen Primary Care Centers are not currently taking online registration. Call the number listed for scheduling and preregistration.
