Palliative Care

Services

The role of the Palliative Care staff is to work with the patient, the family and the patient's health care providers to evaluate treatment options, identify support systems and assist with care coordination between the multiple disciplines serving the patient.

Choices are documented and reviewed by patients, families and all associated care providers so that preferences, values and culture are respected while providing the most beneficial medical treatment.

Palliative care staff members provide:

  • Three to five consultative visits
  • Assessment of care needs wherever the patient resides
  • Coordination of care
  • Care planning through patterned decision-making and development of a common language
  • Volunteer services
  • Communication with health plans
  • Referral to extended services

Palliative care staff members are:

  • Professional social workers, nurses and physicians skilled in palliative care
  • Trained volunteers
