Evergreen Primary Care is affiliated with many health maintenance organizations (HMOs), preferred provider organizations (PPOs) and specialized health plans. Ask the staff at each Evergreen primary care location about the plans with which they participate. They can also tell you which insurance companies will directly bill on your behalf.
