Evergreen’s Rehabilitation Medicine Clinic offers medical evaluation and management of rehabilitation care for patients with neuromusculoskeletal disorders. The clinic offers coordinated, outpatient rehabilitation services to improve function and quality of life.
Rehabilitation nursing offers 24/7 care for physical needs, medication administration, ongoing assessment of medical condition and any necessary intervention. We work collaboratively to reinforce safety standards for mobility, swallowing, fall prevention and skin care. We provide patient/family training including bowel/bladder management, a self-medication program, fluid balance and nutritional needs.
Physical therapists facilitate recovery and independence by maximizing functional mobility and overall safety during mobility. They assist and educate each person on proper body mechanics, increasing their strength and endurance, and improving coordination, balance and range of motion.
Occupational therapists assist patients and teach them how to regain their independence with activities of daily living such as bed mobility, feeding, grooming, dressing, toileting and bathing. They also focus on cognition, upper-extremity strengthening and motor functioning, as well as return-to-work tasks.
Speech pathologists evaluate and treat disorders of speech, language (including verbal and auditory comprehension and expression), reading comprehension, written expression, swallowing, cognition and social interaction. Caregiver training s often an essential component.
Dietitians and dietary technicians monitor the patient's nutritional intake and special dietary needs or modifications. They work with the team, patients and caregivers on diet recommendations and education.
A social worker works closely with patients, their families/caregivers and our interdisciplinary team to develop an appropriate plan for the patient after discharge. The social worker coordinates family conferences and provides assistance in coping with the impact of the patient's illness or injury.

We offer a peer mentor program for those who choose to participate. Speak with past patients who have undergone similar injuries and illnesses, and gain another perspective on how to improve your physical well-being and expand your support.
