Specialized Care

Complex Medical Care for elders who are frail and who have multiple medical conditions. We provide comprehensive assessments to evaluate each patient's health status, including functional and cognitive ability, emotional health and support needs.

Memory Loss and Dementia Assessment which includes comprehensive evaluations by the physician, pharmacist, social worker and nurses to diagnose problems and develop a treatment plan for patients and their families.

Care Coordination with linkage to programs such as home health, health enhancement, hospice, nursing homes, assisted-living facilities, adult family homes and a variety of social services.

Disease Management including anticoagulation therapy, congestive heart failure and diabetes care utilizing a team approach.

Education for individual and families on special topics such as living with chronic diseases, managing difficult behaviors and social problems associated with dementia, and making thoughtful end-of-life decisions.

Insurance

We accept Medicare and Medicaid and most supplemental insurance plans. For your convenience, we will handle billing your insurance for you.
