Prior to your first visit to the Senior Health Specialists, please:

• Read this Welcome Letter
• Print and complete the Pre-Visit Questionnaire

This will save time, and allow us to spend the valuable time with you in your appointment and not filling out paperwork.

Please bring the following items to your initial appointment:

• Completed Pre-Visit Questionnaire
• All medications that you are taking (including vitamins, herbal and other non-prescriptive remedies)
• Insurance information
• Eye glasses and hearing aides (if you have them)
• Old medical records
• Advanced directives (if you have them)
