When Your Surgery is Scheduled


Patient Health Assessment

Your surgeon's office will provide you with a copy of the Patient Health Assessment; it is essential that you complete this before you leave the surgeon's office. If this isn't possible, you can complete the Patient Health Assessment at the Preop Clinic (1-Blue at Evergreen Hospital) on the day you agree to your surgery.

This important questionnaire provides us with a focused view of your health and health history, and helps our preop nurses to determine the necessary pre-operative tests and any specialty consults you will need prior to surgery.

Please be sure to have your full legal name and your correct contact phone number on the Patient Health Assessment; this is the number we will call for your pre-surgical interview.

The Patient Health Assessment will ask for a list of all prescription and over the counter medications you are taking, including vitamins, herbals and supplements. If you are unable to complete this information in your surgeon's office, please have a complete list available for your pre-surgical phone interview. Some medications may be suspended prior to the day of your surgery, as determined by your surgeon and the Preop Clinic.


Pre-Surgical Interview

A Preop nurse will call you within a week of your surgery after reviewing your Patient Health Assessment. You may be called sooner if you have medical conditions requiring consultations prior to surgery. Our pre-surgical nurses will evaluate your Patient Health Assessment, and then conduct a telephone interview to let you know what pre-operative tests and specialty consults you may need prior to your surgery.

If you have not received a phone call 72 hours prior to your surgery, contact the Preop Clinic at 425.899.2706. The Clinic is open weekdays 9 a.m.-5 p.m., closed weekends and holidays.

Your pre-operative instructions will include:

  • A testing plan when indicated
  • Eating and drinking Instructions
  • Medication instructions
  • Information about your procedure and care expectations
  • Any bowel preparations or special diet instructions will come from your surgeon's office
  • Outpatients will need to have a ride home and a care-giver present for the first 24 hours after surgery

Insurance Pre-Authorization

Call Evergreen Hospital preregistration (425.899.2722) to provide your insurance information so we can begin the authorization process.

Please call between 7 a.m.-8 p.m., Monday through Friday. We are closed holidays.
