Rights and Responsibilities

We have adopted these Patient & Client Rights and Responsibilities to help guide our actions. The statement of rights and responsibilities is for those who receive services from Evergreen.

In addition to these rights, we make every effort to protect your privacy and the privacy of your health information. For more information about Evergreen Hospital's privacy practices and your rights regarding privacy, please read our Notice of Privacy Practices.

How to Express Your Concerns

Patients, clients, families and visitors are encouraged to express complaints or concerns about any aspect of their care or experience with Evergreen Hospital.

Concerns or complaints may be made to any Evergreen staff member. Please be assured that expressing a complaint or a concern will not compromise your care. You may also lodge a grievance directly with the Washington State Department of Health and/or the Joint Commission on Accreditation or Healthcare Organizations (JCAHO).

We will promptly investigate all complaints and grievances and work to resolves them in a timely, reasonable and consistent manner, generally within seven (7) days.
