If you are living with blood clots or blood clot disorders and are taking Warfarin (Coumadin), your provider may refer you to the Anticoagulation Clinic for help managing your medication to prevent bleeding or clotting complications.
Common reasons you may be taking Warfarin or Coumadin include:
- Deep Vein Thrombosis
- Pulmonary Embolism
- Certain types of Stroke
- Atrial Fibrillation
In all cases, regular measuring of INR (international normalized ratio), blood clotting time and careful dosage adjustment is critical to prevent complications.
Our Program
The Anticoagulation Clinic is staffed by Registered Pharmacists and Doctors of Pharmacy who are highly trained and skilled members of the health care team. Pharmacists are experts in medication therapies, monitoring medication results, adjusting dosages and preventing dangerous drug interactions.
At each visit, the pharmacist will:
Ask about your medical history and your medications. There are numerous medications that can affect the way Coumadin® works in your body, so it is very important that we know about everything you take. In order to assist us, please bring in a list of all the medications you are currently taking, including both prescription and non-prescription medications. You should call the clinic if you begin taking new medications, including those you purchase without a prescription.
Take a drop of blood from your finger to test the clotting time. The results are obtained immediately, so your Coumadin dosage can be adjusted right away to keep the clotting time within an optimum range. The results and any dosage adjustments are faxed to your physician to keep them fully informed.
When your dosage is stabilized, routine monitoring will take place every four weeks. Since each individual responds differently to Coumadin, some people may need more frequent visits to determine their correct dosage.
Some patients will take blood thinners for a period of time, and others for years or lifelong.
