The growing population of chronically ill people living with progressive, complex, life-limiting conditions presents challenges to our present system of medical care.
Too often this population experiences an extended gap in home care services and repetitive, costly readmissions to the hospital.
- A patient may be actively involved in curative care but not be homebound (excluding them from home care services)
- A patient may have a terminal diagnosis but prefer to continue curative treatment (excluding them from hospice)
The Palliative Care clinicians will consult with the patient, the family and the patient's professional health care providers to evaluate treatment options, identify support systems, and assist with care coordination between the multiple disciplines serving the patient.
When can Palliative Care begin?
Palliative Care can begin at the time of diagnosis and be offered with curative care. Over the course of three to five visits the goals of care will focus on:
- Counseling to help the patient and family adjust to the life-limiting diagnosis
- Assisting patients and families in understanding the patient's condition and options for care
- Providing a consistent and organized decision-making framework
- Optimizing communication between all persons involved with the care of the patient
- Providing physical and psychosocial pain and symptom relief
- Trained volunteers offer a compassionate presence for patients, and respite for families
